The Medicines Optimisation Team aims to ensure that patients within Medway get the medicines they need to achieve the best health outcomes for both the individual and the local community within the resources available.
To improve the quality of medicines use in Medway, closer working between professionals and patients is required. The Medicines Optimisation Team is supported by GP advisers, nurses, the Local Pharmaceutical Committee and patient representatives. The Team work across commissioning pathways and geographic boundaries with local practices, pharmacies, hospitals and other healthcare providers as well as neighbouring Clinical Commissioning Groups.
Effective prescribing and the optimal use of medicines within Medway can:
- Improve primary prevention, for example, controlling blood pressure and cholesterol can prevent patients from having heart attacks and strokes which helps to keep the population healthy in future years reducing medical, economic and social burdens.
- Improve secondary prevention, for example, appropriate prescribing for long term conditions such as diabetes, respiratory and cardiovascular which will reduce disease progression, keeping people healthier, mobile and independent, allowing them to continue to contribute to our local society.
- Ensure legal, safe and clinically effective use of medicines that meet our local health needs and provide value for money.
